SEC: 9.0-12.3; INR: 0.8-1.2 Moderate Intensity Coumadin INR = 2.0-3.0.
Adjustments in anticoagulant therapy dose should be based upon the INR and not the PT in seconds. The INR is used only on patients on stable oral anticoagulant therapy. It makes no significant contribution to the diagnosis or treatment of patients whose PT is prolonged for other reasons.
